A team of experts in post-stroke neurorehabilitation confirmed that the inclusion of prism adaptation treatment in standard care for patients with post-stroke spatial neglect improved functional and cognitive outcomes according to the Functional Independence Measure. The article “Prism adaptation treatment improves the outcome of rehabilitation of hospitalized patients in individuals with spatial neglect: a combined retrospective control study” was published in Rehabilitation and Clinical Translation Research Archives in May 2021.
The authors are Peii Chen, Ph.D., and Emma Kaplan, BS, of the Kessler Foundation’s Stroke Rehabilitation Research Center; Nicole Diaz-Segarra, MD, of the Kessler Rehabilitation Institute; Kimberly Hreha, EdD, OTR / L, of the medical branch of the University of Texas; and AM Barrett, MD, of the Atlanta VA Health Care System. Dr. Chen also has an academic appointment at Rutgers New Jersey Medical School and Dr. Barrett has an academic appointment at Emory University School of Medicine.
Recent research by Dr. Chen’s group shows that after a stroke, approximately 30% of people experience spatial neglect, a neurological disorder that alters a person’s “internal GPS”, making it difficult to navigate. for its environment. Spatial neglect can also affect people with other types of brain injuries. Symptoms include poor balance and navigation, as well as impaired reading and memory. In addition, spatial neglect slows the progress of rehabilitation and functional recovery and increases the risk of injury.
Prism adaptation treatment is a very promising therapy for space abandonment, with some studies indicating that it is beneficial effects it can last for years. During the adaptation of the prism, people wear glasses equipped with optical prisms that move their motor movements to the sloppy side. However, prism adaptation treatment has only been tested as an adjunctive therapy, with little data available on its effectiveness when integrated into the standard of care for people with spatial neglect.
In this study, the researchers analyzed data from patients with spatial neglect at 14 rehabilitation hospitals where the Kessler Foundation’s (KF-PAT) prism adaptation treatment was implemented in occupational therapy. They compared the outcomes of patients who received 8 to 12 daily sessions of prism adaptation treatment with patients who were not treated. The main measurable outcome was the Functional Independence Measure (FIM) and the secondary measurable was the download destination.
“Our results clearly showed that prism adaptation treatment improves the outcome of rehabilitation,” said Dr. Chen, a senior researcher at the Kessler Foundation. “The treated group showed reliably higher scores than the untreated group in total functional independence and cognitive functional independence.” He adds: “This is an extremely encouraging test that integrates the adaptation of the prism in the standard of care for people with spaces negligence it is beneficial “.
The results of the study showed no significant effect on the rate of return home after discharge, and the authors note that there is more research on prism adaptation treatment is needed in this population to further clarify how to optimize its effectiveness.
