Staff at Barking, Havering and Redbridge University NHS Trust reported improvements in care and efficiency after it was put into operation with a healthcare coordination and referral component.
The trust took a phased approach in implementing System C’s CareFlow healthcare coordination application, which is part of its CareFlow EPR, which initially launched it into the multidisciplinary stroke team and tissue viability nurses to support collaboration between teams and between them. Application features include messaging, clinical photos, patient lists, task management, delivery, and referrals.
Mobile electronic references have now replaced paper processes. It used to take up to two days to receive a referral, but now the process is significantly safer and faster.
Debbie Wickens, the trust’s chief tissue viability nurse, said: “Careflow Connect is revolutionizing the way we work. We can see the details of the references, look at the delivery information and the photo of the wound, give priority to patients, create a care plan and make arrangements to see the patient if necessary, all within five minutes. .
Despite the application, staff are able to work more efficiently, reserving initial face-to-face reviews for those who actually require them, while intensifying room staff support activity by conducting more follow-up activities on patients.
The use of task management, with tasks assigned electronically according to the set of competencies within the team, has also improved efficiency and, by eliminating paper, security.
Through the CareFlow app, multidisciplinary teams can coordinate the care they provide. On the stroke team, for example, nurses, AHPs, doctors, physiotherapists, and language therapists would use the same app, leveraging their smart lists and messaging features, as well as users ’ability to share. rich clinical information through delivery profiles.
Confidence is now deploying the application more widely for care for the elderly, orthopedics and vascular equipment, which also incorporates allied health professionals, pharmacy staff and clinical specialist nurses.
Barking, Havering and Redbridge will also develop a number of additional EPR features over the coming months in a program designed to create digital patient pathways and support clinical teams.
