An enhanced elbow MRI view for Major League Baseball pitchers

According to a pilot study published in ARRS ‘ American Journal of Roentgenology (AJR), the external rotational vision of the flexed valgus elbow (FEVER) may improve the MRI of the ulnar collateral ligament (UCL) in Major League Baseball (MLB) pitchers.

“The width of the joint space confirms the stress of the valgus elbow with the FEVER sight,” wrote the corresponding author Thomas Knoblauch at the University of Nevada in Las Vegas. “Diagnostic confidence increased and additional UCLs were identified as abnormal.”

Due to the repetitive stress of extreme valgus during aerial throwing maneuvers, UCL injuries remain common in throwing athletes. Because standard positioning for elbow MRI is often not optimal for UCL representation, Knoblauch, Arizona Diamondbacks team physician Gary Waslewski and colleagues piloted his study to evaluate the impact of FEVER vision on the measurement of ulnotroclear joint space (UT) and the evaluation of the UCL reader, as incorporated into conventional MRI of the elbow.

A total of 44 MLB launchers underwent magnetic resonance imaging of the elbow, including standard sequences and a weighted sequence of coronal fat saturated proton density in the FEVER sight using specific positioning maneuvers and sandbags to immobilize the elbow. in valgus voltage, while the UCL parallel to the long axis was displayed. To measure the joint UT space and assess confidence in UCL-related findings with general UCL normality, two radiologists independently assessed the standard and FEVER views.

“Among the 44 MLB pitchers undergoing MRI of the elbow,” Knoblauch et al. continued, “the FEVER view, compared to the standard view, generated increased UT joint space width (mean magnification, 1.80 mm) and increased confidence for three of the five UCL-related findings. (average increase, -0.14 to 0.98) “. Readers 1 and 2 rated additional 3 and 2 UCLs as abnormal in FEVER compared to standard view; no reader has rated any UCL as abnormal in the standard view, but normal in the FEVER view.
